Copays are due at the time of service. Please check with your insurance benefits to determine your copayment. It is a patient’s responsibility to know their own insurance benefits. We do not bill for the hospital or labwork that may be ordered during your visit to us. If you are scheduled for a procedure or labwork you will receive a separate bill. If your insurance requires a referral or prior authorization, you must have it at the time of service. Your appointment will need to be rescheduled if prior authorization is not obtained.

While we gladly provide this service to our patients, it is important for the patient to be familiar with the guidelines of their insurance plan requirements regarding authorizations, deductibles, co-payments and other vital requirements.
